On cardiac output specifically: Swan-Ganz uses bolus thermodilution (cold saline injected in the right atrium, temperature change measured in the PA) intermittently, or continuous thermodilution catheters that use a heating filament. PiCCO calibrates an intermittent transpulmonary thermodilution measurement (injectate through the central line, sensed at the arterial catheter tip) against the arterial pulse contour, then extrapolates continuous beat-to-beat cardiac output from the arterial waveform between calibrations. This gives PiCCO the advantage of truly continuous real-time trending without needing repeated boluses, but the accuracy depends entirely on how good that pulse contour algorithm is between calibrations.

Pitfalls: Swan-Ganz carries the bigger procedural risk profile, arrhythmias during insertion, PA rupture, catheter knotting, and PCWP readings can be badly misread if not properly zeroed, leveled, or if the balloon is over-wedged or the patient is on high PEEP. Thermodilution CO measurements from either device get thrown off by tricuspid regurgitation, intracardiac shunts, irregular rhythms like atrial fibrillation, and rapid fluid shifts. PiCCO's pulse contour analysis drifts and becomes unreliable with vasopressor changes, aortic aneurysm or severe peripheral vascular disease, intra-aortic balloon pumps, and arrhythmias, so it needs frequent recalibration in an unstable patient, and it can't give you PA pressures, PCWP, or mixed venous sat directly since there's no catheter in the pulmonary circulation, so it's blind to isolated right heart or pulmonary vascular problems that Swan-Ganz would catch immediately.
